WAIVER FORM
TO BE SIGNED BY ALL PARTICIPANTS
(INCLUDING PEOPLE RIDING ON THE FLOAT)
I knowingly and at my own risk and in consideration of my acceptance as a participant, hereby forever waive and release any and all claims for injuries or illness that I may incur as a direct or indirect result of my participation in the Colorado Ice Fest "Lighter Side of Christmas Parade," against Special Edition-Colorado Ice Fest, Colorado Ice Artists and Education, Inc., The City of Woodland Park, any sponsor or any other group or individual sustaining or associated with this Parade. I acknowledge that I have read, understand, and accept my responsibility and liability for my or that of my child’s safety and health during the Parade.
